5. Your Right to Access Dental Records
Under the Health Insurance Portability and Accountability Act (HIPAA) and North Carolina law, you have the legal right to access your dental records. This includes:
- Treatment Records: Progress notes, treatment plans, clinical findings, and examination records
- Diagnostic Images: Radiographs (X-rays), CT scans, intraoral photographs, and digital imaging
- Laboratory Records: Dental laboratory work orders, prescriptions, and prosthetic specifications
- Billing Records: Itemized statements, insurance claims, and payment history
Record Request Process: You may request copies of your dental records at any time by contacting our office. We are required by federal law to provide copies within 30 days of your request. A reasonable, cost-based fee may be charged for copying and transferring records as permitted under HIPAA regulations.
Important: Your right to access your dental records cannot be denied due to an outstanding account balance, pending legal matters, or any other reason prohibited under federal HIPAA statutes. Original records remain the property of the dental practice, but you are entitled to receive complete, legible copies of diagnostic quality.
